1. Upon successful payment, you will receive an email confirming your sample collection slot/vaccination appointment and lab test/vaccination specific instructions. If your chosen date and time slot is not available, our team will get in touch to let you know what slots are available and confirm your collection/vaccination appointment.
2. In any of any lockdowns and/or restrictions imposed by the ongoing COVID-19 pandemic, adverse weather conditions or any other unforeseen circumstances, your sample collection slot/vaccination slot might change (in such cases, we will notify you).
3. All bookings are non-refundable except in cases of non-performance of services.
4. Rescheduling of your sample collection slot/vaccination appointment once confirmed by Proactive might attract an additional rescheduling charge
5. The testing will be conducted by an NABL accredited diagnostic service provider. Proactive For Her (PFH) is only a facilitator which facilitates diagnostic services between you and the diagnostic service provider. PFH cannot be held liable in case of any issues during sample collection. The diagnostics service provider is solely liable for or responsible towards the actual test and analysis undertaken and the underlying diagnosis provided in the lab report. PFH hereby disclaims and excludes all warranties with respect to all information and diagnosis contained in the lab report. 6. Vaccines will be administered by our partner Portea. Proactive For Her (PFH) is only a facilitator which facilitates vaccination services between you and the vaccine service provider. PFH cannot be held liable in case of any issues during and after vaccination. The vaccine service provider is solely liable for or responsible for the administration of the vaccine. PFH hereby disclaims and excludes all warranties with respect to any aspect of the vaccine administration.
